Registration Instructions

PRO TIPS:
Make sure your student has a CURRENT sports physical on file. Learn how

Have ready:

• Your student’s school email,
• Primary clinic phone number,
• Doctor’s name, and
• Your student athlete available to answer questions.
  Expect the registration to take 15-20 minutes.

PARENTS/GUARDIANS: Please ensure your athlete’s physical is up to date. If you are not seeing the athletic activities you are expecting, please verify your student’s physical is current (within 3 years of the END DATE of the activity). You can check your student’s physical date in ParentVue: At the bottom of the Student Information tab is their ‘Activity Screening Date’, again, this date needs to be within three years of the END of the sport season.

NEW THIS YEAR: MSHSL Eligibility Brochure and Checklist are included with each enrollment. There is no separate form to complete this year. Expect registration to take 15-20 minutes.

FREE/REDUCED PRICING: If your student qualifies for free lunch, their MSHSL Activity fees will be waived. Students who qualify for reduced lunch will have their MSHSL Activity fees reduced by 50%. You must apply EACH YEAR to receive these benefits. If you are expecting your student’s fee to be reduced or waived, you will see the change AFTER the item is placed in the cart. If you do not see the price adjustment, please contact the school’s athletics office prior to checking out. We encourage all families to fill out the free/reduced form.
